The only way you can find out is to try, but take it slowly. For trail riding she will have been using different muscles to when she was doing dressage, but I should not think she will have much trouble as long as she is sound.
For the jumping, restart with trotting poles, cross poles and build it up from there and being older she might need support boots for safety. She should be fine. I look forward to hearing how she gets on.
Try feeding oil and well moistened feed for her weight problem and try to keep her warm. And soak her hay to make it easier to chew.
